Log Book Submission

Requirements

These requirements are in addition to those mentioned in the regulations for the measuring of Nuchal Translucency, the 11-14 week scan.

  • De-identify all images (thermal, transparencies and computer image files). For thermal paper and transparencies, use scissors to cut out the patient’s name and site of scan. Please note that blocking out details with texta or similar is not appropriate.  On electronic images these details need to be cropped or blacked out.
  • Date and time scan was performed to remain on image.
  • Each image in the logbook is to be from a different patient.
  • Images need to be placed in the logbook in order of date and time.
  • Logbook to contain 5 images.
  • Thermal images pasted onto single A4 page or similar and numbered.
  • Transparencies are to be (sticky)-taped inside a plastic sleeve and numbered. Three to four images per sleeve depending on size. Click here for a sample image.
  • Computer image files should be in .jpg or .jpeg format and be named as “Image 1.jpg”, “Image 2.jpg” and so on. The 5 image files should be burnt on to a CD or emailed to the program along with the operators ID number.
  • Do not identify your logbook with your name. (If you know your ID number record this on the logbook, if not a covering letter / email will suffice and the staff at the Program will record your ID on the logbook / images.) The logbook and images are to be de-identified for the purpose of un-biased and fair assessment.
    • If there are submission transgressions, images will be returned.
